The OMMU Registry or the Office of Medical Marijuana Use is in charge of the Florida medical marijuana treatment program, which includes Florida medical card applications, approvals, and the MMUR also known as the Medical Marijuana Use Registry. The MMUR or Medical Marijuana Use Registry is a state run website and centralized location for information pertaining to medical marijuana in the state of Florida. The MMUR serves as a focal point for law enforcement, Florida marijuana dispensaries, physicians, and most importantly the medical marijuana treatment patients. Patients in the state of Florida will need to access the MMUR to complete their initial application, make payment for their application, process renewal applications, and view current recommendations and order histories at dispensaries. Releaf Medical has provided our patients with details on all events related to the OMMU Florida and the MMUR Florida. 1. New Patient Registration & Application Payment

OMMU Registry Process Map

Step 1: At the time of a patient’s initial appointment a staff member at Releaf Medical will create a patient profile in the MMUR. The patient will provide their social security number, date of birth, Florida drivers license number, email address, and current weight if they are a permanent resident of Florida. A seasonal resident will be asked additional questions when creating a profile in the MMUR. After the profile in the MMUR has been created the patient will receive two emails from the state of Florida. One of the two emails will look similar to the email example displayed below.

Step 2: The patient will open the email from the state of Florida that includes the patient’s “current password.” Patients will copy this current password, making sure that all characters have been copied. If an email is not received immediately please check SPAM folders.

Step 3: Click on the link located at the bottom of the email. This link will direct patients to the Medical Marijuana Use Registry or MMUR.

Step 4: Select the “Login” option. Username will be the email address that was provided during the creation of the profile. The password will be the temporary password which was sent by the state of Florida via email, paste the password that you copied in step 2.

Step 5: Patients will be required to update their current password. The current password will need to be pasted again and a new password will be entered two times. The new password must be 10 characters in length, include lowercase and uppercase letters, a number, and a special character. Special characters are (!”#$%&'()*+,-./:;<=>?@[\]^_`{|}~)

Step 6: Select the “Your Card” option in the menu

Step 7: Select the “Begin Initial Application” option and click “Proceed” on the pop-up

Step 8: Review all the information displayed on the profile details and select the “Apply” option

Step 9: Click “Supply A Photo” to supply your Florida state identification photo or to supply your own passport style photo

Step 10: A pop-up will appear for you to either check for your state ID photo or to supply your own passport style photo. Permanent residents of Florida will select “Check State ID Photo” and this will upload your drivers license photo from the DMV. Seasonal residents of Florida will need to select “Supply My Own” and upload a passport style photo of only their face against a white background. Select the “Proceed” option in the pop-up window.

Step 11: Scroll down below the profile details to the Proof of Residence section of the application. Select the “Supply Proof” option.

Step 12: Permanent residents of the state of Florida will select “Check State Residency Status” to obtain the information on file at the DMV. Seasonal residents will select the “Supply My Own” option and upload two documents with their name and Florida address to prove residency in the state of Florida. Select the “Proceed” option in the pop-up window.

Step 13: Permanent residents will review the information displayed in the profile page again and select the “Apply” option. Seasonal residents will review that their documentation uploaded correctly in the Proof of Residence section of the initial application.

Step 14: Scroll down to the Signature section of the application and type in your first and last name into the spaces provided. Then select “Submit My Card Application.” After selecting this option a pop-up will appear that says “Payment Available,” select the “Close” option.

Step 15: Scroll to the Payment Record section of the application that is located above the Signature section. Select the “Click Here To Pay Online” option.

Step 16: Select the Credit Card tab on the payment screen and enter in the information required, then select “Continue.” Verify your email address on the payment screen, agree to the terms and conditions, and select “Make Payment.”

Step 17: A payment confirmation screen will appear. Releaf Medical advises patients to screenshot this payment page. An email confirmation of payment will also be sent and these records should be kept until the state of Florida provides the patient their temporary Florida medical card. The temporary card will be issued by the state of Florida 5-10 business days after the application and payment have been processed.

Step 16: Select the Credit Card tab on the payment screen and enter in the information required, then select “Continue.” Verify your email address on the payment screen, agree to the terms and conditions, and select “Make Payment.”

Step 17: A payment confirmation screen will appear. Releaf Medical advises patients to screenshot this payment page. An email confirmation of payment will also be sent and these records should be kept until the state of Florida provides the patient their temporary Florida medical card. The temporary card will be issued by the state of Florida 5-10 business days after the application and payment have been processed.

2. Florida Medical Card Online Renewal

To maintain an active MMUR identification card with the OMMU Registry patients and caregivers must submit a renewal application and pay the state of Florida application fee once a year. Renewal applications are available to patients beginning 45 days prior to expiration displayed on the date of their MMUR identification card. The state of Florida will send patients an email reminder as well.

Step 1: Open your web browser and navigate to the MMUR or Medical Marijuana Use Registry.

Step 2: Select the “Login” option. Type in your username, which will be the email address that was provided to the state of Florida, and your password. If you have forgotten your password please read our Change Password or Forgot Password section.

Step 3: Select the “Your Card” option in the menu

Step 4: Select the “Begin Renewal” option and click “Proceed” on the pop-up

Step 5: Review all the information displayed on the profile details and select the “Apply” option

Renewal patients can use their active Florida medical card until the expiration date on their card passes. After this date the temporary Florida medical card issued by the state will have to be used until the permanent card is received.

3. How To Remove Current Physician OMMU Florida

Patients have the right to change their current physician at any time on their MMUR profile with the OMMU Florida. Please follow the instructions below to remove your current physician at the OMMU Registry.

Step 1: Open your web browser and navigate to the MMUR or Medical Marijuana Use Registry.

Step 2: Select the “Login” option. Type in your username, which will be the email address that was provided to the state of Florida, and your password. If you have forgotten your password please read our Change Password or Forgot Password section.

Step 3: Select the “Your Profile” option in the menu

Step 4: Select the “Leave Your Current Physician” option at the bottom of the page under the profile details.

Step 5: Select “Confirm” in the pop-up window. Once this is complete the profile will now display “No Current Physician” under your profile details. See example image below.

4. Manage OMMU Registry Profile Details (Phone & Email)

Patients have the ability to update their contact information in the MMUR profile page, which is phone number and email address. If patients need to update any other information please contact Releaf Medical directly and we will assist with the process. Releaf Medical does not recommend updating your address on the MMUR profile page as the OMMU Florida will make patients complete a new application for the change of address to take place. It is better to update an address during the annual OMMU Registry renewal process.

Step 1: Open your web browser and navigate to the MMUR or Medical Marijuana Use Registry.

Step 2: Select the “Login” option. Type in your username, which will be the email address that was provided to the state of Florida, and your password. If you have forgotten your password please read our Change Password or Forgot Password section.

Step 3: Select the “Your Profile” option in the menu

Step 4: Select the “Manage Profile Details” option at the bottom of the page under the profile details.

Step 5: Select the “Update Phone/Email” option

Step 6: Enter in your new contact information into the fields provided on the web page and select the “Save” option upon completion.

5. Check Florida Medical Card Status OMMU Registry

Patients have the ability to check the status of their Florida medical card with the OMMU Registry. Upon following the instructions below patients will be able to view either a green check mark or a red X on the profile page. A green check mark means the Florida medical card is active with the OMMU Florida and a red check mark means the card is not active with the OMMU Registry. Patients can also select the your card option and view the status of the card, the actual expiration date of the card, and the date when the patient can begin the OMMU renewal process. The state of Florida requires medical marijuana treatment patients in the state of Florida to have both an active Florida medical card and active doctor recommendations. If patients require assistance with understanding the active doctor recommendations please view section 7, View Active Doctor Recommendations. Patients can also fill out the form located on our Card Status web page and a Releaf Medical staff member will review the status of your Florida medical card and contact you with the findings.

Step 3: Select the “Your Card” option in the menu. Your Florida medical card status can be viewed. The following will be visible on the MMUR your card page, status of the card, card expiration date, and the begin renewal application date.

6. Change OMMU Registry Password & Forgot Password

Releaf Medical understands people are human and forget passwords, so does the OMMU Registry and provides patients with the ability to simply update their password for the MMUR. The process of updating your password with the OMMU Florida Registry is very simple, but at Releaf Medical we know that not everyone is comfortable with computers, so call us at 561-778-5323 if you require assistance with a password reset for the MMUR. If a patient does not remember what email address they have registered with the OMMU Registry then they will have to contact the Office of Medical Marijuana Use Directly, their contact information is displayed at the top of this page. Below is the step by step process of how to set a new password in the case of not remembering your existing password. If a patient already has access to the MMUR and requires to change their existing password they would login to the MMUR and select the “Change Password” option in the menu bar. The patient is required by the OMMU Registry to enter in their current password once and then type in a new password two times. After entering in the new password information you would select the “Submit” option and the password will have been changed.

Step 1: Open your web browser and navigate to the MMUR or Medical Marijuana Use Registry.

Step 2: Select the “Login” option.

Step 3: Select the “Forgot Password” option located below the “LogIn” button.

Step 4 : Patients will enter the email address they have registered with the MMUR and select the “Submit” button. If patients do not remember which email they have provided the state then the patient will need to contact the state directly. The contact information for the OMMU Registry is displayed at the top of this page along with their hours of operation.

Step 5 : Patients will receive an email from the OMMU Registry and in this email there will be a hyperlink that says “Click This Link To Reset Your Password.” Patients will select this hyperlink option.

Step 6 : Patients will be required to update their password. The new password will need to be entered two times. The new password must be 10 characters in length, include lowercase and uppercase letters, a number, and a special character. Special characters are (!”#$%&'()*+,-./:;<=>?@[\]^_`{|}~). After typing in the new OMMU Registry password two times patients will select the “Submit” option.

Step 7 : Patients will select the “Log In” option in the menu bar and type in their updated login credentials.

7. View Active Doctor Recommendations & Purchase History

After a patient has successfully logged into the Medical Marijuana Use Registry website they will select the “Your Profile” option on the menu bar. Once a patient is on the “Your Profile” page they will scroll down below the profile details and can view an “Orders” section. The orders are a patient’s active recommendations and previous recommendations with the state of Florida. Medical marijuana treatment patients will be able to view the order status, order type, start date, and end day for each specific delivery route. Typically a patient will see options for Medical Marijuana, Low-THC Cannabis, and Smoking Marijuana. Patients are able to see the status of these orders as they will appear as, open, scheduled, completed, or expired. An open order allows the medical marijuana treatment patient to purchase products at Florida dispensaries. Scheduled orders are for future orders to be placed upon the completion of an existing open order. Completed orders are when the patient has used all of the allowance for a specific delivery route. Expired orders are past recommendations that are no longer active with the state of Florida. If a patient logs into the Medical Marijuana Use Registry and views only expired orders that patient will need to have a re-evaluation appointment with their marijuana doctor. The maximum amount of time any marijuana doctor in the state of Florida can provide a recommendation for the use of medical marijuana is 210 days, 7 months.

Step 2: Select the “Login” option and sign into the MMUR profile.

Step 3: Select the “Your Profile” menu option and scroll down to below the profile details section.

Step 4: Reading and understanding an order or recommendation sheet. Marijuana doctors will place recommendations for marijuana products and THC milligram limitations on specific categories. These categories are Smoking Marijuana, Oral, Edibles, Topical, Sublingual, Inhalation, and Suppository. Each specific delivery method will have a determined amount of THC that can be purchased within a specific period of time. Only a patient’s marijuana doctor can adjust the delivery route amounts. The OMMU Registry has placed a limit of 2.5 ounces of smokeable flower every 35 days, this is a state of Florida limit and can not be increased. Patients can select the “Expand” option and view the specific allowances and product history for each individual delivery route. Smokable flower details will be under the order type of “ Smoking Marijuana” and all other delivery routes can be viewed under the “Medical Marijuana” order type. When viewing the Medical Marijuana order type there will be sub tabs for each delivery route.
